Old-Age / Invalidity Pension form

The form is in Urdu. It asks for name, CNIC, spouse, address, EOBI registration no, date of birth, joining date, leaving date and employers.

• Page 2 has the employer certificate. The employer signs and stamps it.
• It also has the claimant’s affidavit (bayan-e-halfi).
• Applying more than one year late? Fill the delay part and give the reason.
• Documents to attach

Survivor’s Pension form

• For the widow or family of a deceased insured person or pensioner.
• Needs the deceased’s CNIC, EOBI No, death date and death certificate details.
• Includes an affidavit and a succession statement (Warasat Nama).
• The succession statement must be attested by a grade 17 officer or a Union Council chairman.

PR-01 employer registration form

• For employers registering an establishment with EOBI.
• Required within 30 days of the Act applying (5 or more employees).
• Employers can also register online: Register Employer.

How to submit

1. Print the form and fill it in clear writing.
2. Attach attested copies of documents.
3. Submit at your regional office.
4. Keep the Claim Form No or receipt.
